Fate of a Morselized Impaction Allograft Used for the Defect of Acetabulum in Revision Total Hip Arthroplasty

Abstract
Purpose: To find out whether or not the hips, followed more than 6 years after revision THA using the morselized impaction allograft for filling bone defects around the acetabulm, are well-maintained without a bone loss.

Materials and Methods: The study included 77 cases (81 hips) who were followed at least 6 years (range, 6-12 years and 10 months) at our hospital. In all cases, the mosrselized impaction allografts were and the maximum diameter acetabular component were used. For evaluation, the magnified anteroposterior, and lateral radiolographs, those made at immediate postoperation and the latest follow-up, made at our center were analyzed.

Results: Except for one revisional case attributed to a fixation loss, in the rest of cases, a firm fixation was observed. There were 32 cases in which radiodensity of the graft increased as compared with that in the immediate postoperative state in the latest follow-up radiographs. In 48 cases, a decrease in radiodensity was noted, and there were 2 cases showing the radiolucent zones between the host bone and the graft and 9 cases of the periacetabular radiolucent zones, both being less than 2-mm.

Conclusion: The morselized impaction allografts used to fill the bone defects around the acetabulum in revision THA showed a satisfactory result. Since the result yielded up to this point is satisfactory, although a longer period of follow-up is desirable, it may be considered as an alternative means of resolving the periacetabular bone defects in revision THA.
